Pojedinci sa gojazno\u0161\u0107u \u010desto imaju izmenjene nivoe tiroidnih hormona. Pacijenti sa autoimunskim bolestima \u0161titne \u017elezde \u010desto imaju i druga stanja povezana sa imunitetom.  <\/p>\n\n<p class=\"wp-block-paragraph\">Ali da li su ovi odnosi zaista uzro\u010dni, ili su to jednostavno korelacije pod uticajem drugih faktora kao \u0161to su \u017eivotni stil, starost, ishrana ili osnovna zdravstvena stanja?<\/p>\n\n<p class=\"wp-block-paragraph\">Razlikovanje korelacije od uzro\u010dnosti jedan je od najve\u0107ih izazova u medicinskim istra\u017eivanjima. Me\u0111utim, ove asocijacije mogu biti pod uticajem konfuzornih faktora ili obrnute uzro\u010dnosti. <\/p>\n\n<p class=\"wp-block-paragraph\">Zamisli da istra\u017eiva\u010di primete da ljudi sa ni\u017eim nivoom tiroidnih hormona tako\u0111e imaju ve\u0107i rizik od kardiovaskularnih bolesti. Da li smanjena funkcija \u0161titaste \u017elezde pove\u0107ava kardiovaskularni rizik? Ili drugi faktori, kao \u0161to su gojaznost, fizi\u010dka neaktivnost, upotreba lekova ili hroni\u010dne bolesti, uti\u010du i na funkciju \u0161titaste \u017elezde i na bolesti srca?  <\/p>\n\n<p class=\"wp-block-paragraph\">Sli\u010dno tome, ako pacijenti sa odre\u0111enom bole\u0161\u0107u imaju ni\u017ee koncentracije vitamina D, ne mo\u017eemo odmah zaklju\u010diti da je nedostatak vitamina D uzrokovao bolest. Sama bolest mo\u017ee smanjiti nivo vitamina D, ili na oba mogu uticati potpuno razli\u010diti faktori. <\/p>\n\n<p class=\"wp-block-paragraph\">Randomizovane kontrolisane studije (RKS) smatraju se zlatnim standardom za utvr\u0111ivanje uzro\u010dnosti jer se u\u010desnici nasumi\u010dno raspore\u0111uju u razli\u010dite intervencije. Na\u017ealost, RKS su \u010desto skupe, dugotrajne, a ponekad ih je nemogu\u0107e ili neeti\u010dno sprovesti. <\/p>\n\n<p class=\"wp-block-paragraph\">Mendelska randomizacija nudi elegantnu alternativu.<\/p>\n\n<h2 class=\"wp-block-heading\">\u0160ta je Mendelska randomizacija?<\/h2>\n\n<p class=\"wp-block-paragraph\">Mendelska randomizacija je statisti\u010dka metoda koja koristi prirodno nastale genetske varijacije za istra\u017eivanje da li je uo\u010dena asocijacija izme\u0111u faktora rizika i bolesti verovatno uzro\u010dna.<\/p>\n\n<p class=\"wp-block-paragraph\">Metoda se zasniva na jednostavnom biolo\u0161kom principu koji je prvi opisao Gregor Mendel: genetske varijante se nasumi\u010dno nasle\u0111uju od roditelja pri za\u010de\u0107u. Ova prirodna nasumi\u010dna alokacija podse\u0107a na proces randomizacije koji se koristi u klini\u010dkim ispitivanjima. <\/p>\n\n<p class=\"wp-block-paragraph\">Ovaj koncept je ilustrovan na Slici 1. Dok su u\u010desnici u randomizovanim kontrolisanim studijama nasumi\u010dno raspore\u0111eni u intervencione ili kontrolne grupe od strane istra\u017eiva\u010da, pojedinci u studijama Mendelske randomizacije su prirodno \u201ealocirani\u201c na razli\u010dite nivoe izlo\u017eenosti kroz nasumi\u010dno nasle\u0111ivanje genetskih varijanti pri za\u010de\u0107u. <\/p>\n\n<p class=\"wp-block-paragraph\">Ova prirodna randomizacija poma\u017ee u minimiziranju konfuzije i omogu\u0107ava istra\u017eiva\u010dima da ispitaju uzro\u010dne veze koriste\u0107i opservacione podatke.<\/p>\n\n<p class=\"wp-block-paragraph\">Budu\u0107i da su genetske varijante dodeljene pre ro\u0111enja, na njih generalno ne uti\u010du \u017eivotni stil, izlo\u017eenost okolini, progresija bolesti ili ve\u0107ina konfuzornih faktora koji se sre\u0107u u opservacionim studijama.<\/p>\n\n<p class=\"wp-block-paragraph\">U Mendelskoj randomizaciji, genetske varijante povezane sa izlo\u017eeno\u0161\u0107u slu\u017ee kao instrumentalne varijable (ili genetski instrumenti) koje omogu\u0107avaju istra\u017eiva\u010dima da procene uzro\u010dni efekat te izlo\u017eenosti na ishod bolesti.<\/p>\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"908\" height=\"580\" src=\"https:\/\/www.innothyrogen.eu\/wp-content\/uploads\/2026\/07\/image-1.png\" alt=\"\" class=\"wp-image-2805\" srcset=\"https:\/\/www.innothyrogen.eu\/wp-content\/uploads\/2026\/07\/image-1.png 908w, https:\/\/www.innothyrogen.eu\/wp-content\/uploads\/2026\/07\/image-1-300x192.png 300w, https:\/\/www.innothyrogen.eu\/wp-content\/uploads\/2026\/07\/image-1-768x491.png 768w\" sizes=\"(max-width: 908px) 100vw, 908px\" \/><\/figure>\n\n<p class=\"has-text-align-center wp-block-paragraph\"><strong>Slika 1.<\/strong> <em>Mendelska randomizacija kao prirodni analog randomizovane kontrolisane studije. U Mendelskoj randomizaciji, nasumi\u010dna alokacija genetskih varijanti pri za\u010de\u0107u stvara grupe koje su generalno uravnote\u017eene u pogledu konfuzornih faktora, omogu\u0107avaju\u0107i istra\u017eivanje uzro\u010dnih veza kori\u0161\u0107enjem opservacionih podataka. Adaptirano iz Davey Smith &amp; Ebrahim (2005).<\/em><\/p>\n\n<h2 class=\"wp-block-heading\">Kako funkcioni\u0161e Mendelska randomizacija?<\/h2>\n\n<p class=\"wp-block-paragraph\">Osnovni koncept je jednostavan.<\/p>\n\n<p class=\"wp-block-paragraph\">Prvo, istra\u017eiva\u010di identifikuju genetske varijante koje su sna\u017eno povezane sa specifi\u010dnom izlo\u017eeno\u0161\u0107u, kao \u0161to su:<\/p>\n\n<ul class=\"wp-block-list\">\n<li>tireostimuli\u0161u\u0107i hormon (TSH)<\/li>\n\n\n\n<li>slobodni tiroksin (fT4)<\/li>\n\n\n\n<li>nivoi vitamina D<\/li>\n\n\n\n<li>indeks telesne mase<\/li>\n\n\n\n<li>koncentracije holesterola<\/li>\n\n\n\n<li>inflamatorni biomarkeri<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\">Ove varijante se identifikuju kroz velike studije asocijacije na nivou celog genoma (GWAS), \u010desto uklju\u010duju\u0107i stotine hiljada u\u010desnika.<\/p>\n\n<p class=\"wp-block-paragraph\">Zatim, istra\u017eiva\u010di ispituju da li su iste genetske varijante tako\u0111e povezane sa ishodom bolesti.<\/p>\n\n<p class=\"wp-block-paragraph\">Ako genetske varijante koje pove\u0107avaju (ili smanjuju) izlo\u017eenost dosledno dovode do ve\u0107eg (ili manjeg) rizika od bolesti, to pru\u017ea dokaz koji podr\u017eava uzro\u010dnu vezu.<\/p>\n\n<p class=\"wp-block-paragraph\">Drugim re\u010dima, umesto da se pita:<\/p>\n\n<p class=\"wp-block-paragraph\"><em>\u201eDa li ljudi sa vi\u0161im TSH \u010de\u0161\u0107e razvijaju bolest?\u201c<\/em><\/p>\n\n<p class=\"wp-block-paragraph\">MR pita:<\/p>\n\n<p class=\"wp-block-paragraph\"><em>\u201eDa li ljudi koji su genetski predisponirani da imaju vi\u0161i TSH tokom \u017eivota tako\u0111e imaju druga\u010diji rizik od razvoja bolesti?\u201c<\/em><\/p>\n\n<figure class=\"wp-block-image size-large\"><img decoding=\"async\" width=\"1024\" height=\"569\" src=\"https:\/\/www.innothyrogen.eu\/wp-content\/uploads\/2026\/07\/test-tube-containing-dna-1024x569.jpg\" alt=\"\" class=\"wp-image-2809\" srcset=\"https:\/\/www.innothyrogen.eu\/wp-content\/uploads\/2026\/07\/test-tube-containing-dna-1024x569.jpg 1024w, https:\/\/www.innothyrogen.eu\/wp-content\/uploads\/2026\/07\/test-tube-containing-dna-300x167.jpg 300w, https:\/\/www.innothyrogen.eu\/wp-content\/uploads\/2026\/07\/test-tube-containing-dna-768x427.jpg 768w, https:\/\/www.innothyrogen.eu\/wp-content\/uploads\/2026\/07\/test-tube-containing-dna-1536x853.jpg 1536w, https:\/\/www.innothyrogen.eu\/wp-content\/uploads\/2026\/07\/test-tube-containing-dna-2048x1138.jpg 2048w\" sizes=\"(max-width: 1024px) 100vw, 1024px\" \/><\/figure>\n\n<h2 class=\"wp-block-heading\">Za\u0161to je Mendelska randomizacija toliko mo\u0107na?<\/h2>\n\n<p class=\"wp-block-paragraph\">Jedna od glavnih prednosti MR je njena sposobnost da smanji pristrasnost.<\/p>\n\n<p class=\"wp-block-paragraph\">Budu\u0107i da se genetske varijante nasle\u0111uju pre ro\u0111enja:<\/p>\n\n<ul class=\"wp-block-list\">\n<li>obi\u010dno se ne mogu promeniti bole\u0161\u0107u,<\/li>\n\n\n\n<li>nezavisne su od faktora \u017eivotnog stila,<\/li>\n\n\n\n<li>manje su pod uticajem konfuzije nego konvencionalne opservacione studije.<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\">Kao rezultat toga, MR \u010desto pru\u017ea ja\u010de dokaze o uzro\u010dnosti nego samo opservaciono istra\u017eivanje.<\/p>\n\n<p class=\"wp-block-paragraph\">Va\u017eno je napomenuti da MR ne zamenjuje u potpunosti randomizovane klini\u010dke studije. Umesto toga, dopunjuje ih poma\u017eu\u0107i istra\u017eiva\u010dima da prioritizuju koji su biolo\u0161ki putevi najperspektivniji za dalja istra\u017eivanja i potencijalnu terapijsku intervenciju. <\/p>\n\n<h2 class=\"wp-block-heading\">Tri osnovne pretpostavke<\/h2>\n\n<p class=\"wp-block-paragraph\">Da bi Mendelska randomizacija pru\u017eila pouzdane uzro\u010dne procene, moraju biti ispunjene tri klju\u010dne pretpostavke.<\/p>\n\n<h3 class=\"wp-block-heading\">1. Relevantnost<\/h3>\n\n<p class=\"wp-block-paragraph\">Genetske varijante moraju biti sna\u017eno povezane sa izlo\u017eeno\u0161\u0107u od interesa.<\/p>\n\n<h3 class=\"wp-block-heading\">2. Nezavisnost<\/h3>\n\n<p class=\"wp-block-paragraph\">Genetske varijante ne bi trebalo da budu povezane sa konfuzornim faktorima koji uti\u010du i na izlo\u017eenost i na bolest.<\/p>\n\n<h3 class=\"wp-block-heading\">3. Ograni\u010denje isklju\u010denja<\/h3>\n\n<p class=\"wp-block-paragraph\">Genetske varijante bi trebalo da uti\u010du na bolest samo kroz izlo\u017eenost koja se prou\u010dava, a ne kroz alternativne biolo\u0161ke puteve.<\/p>\n\n<p class=\"wp-block-paragraph\">Istra\u017eiva\u010di rutinski procenjuju ove pretpostavke koriste\u0107i niz statisti\u010dkih analiza osetljivosti, poma\u017eu\u0107i da se osigura da su zaklju\u010dci \u0161to robusniji.<\/p>\n\n<h2 class=\"wp-block-heading\">Mendelska randomizacija u istra\u017eivanju \u0161titne \u017elezde<\/h2>\n\n<p class=\"wp-block-paragraph\">Mendelska randomizacija je postala sve vredniji alat za razumevanje biologije \u0161titne \u017elezde.<\/p>\n\n<p class=\"wp-block-paragraph\">Istra\u017eiva\u010di su primenili MR za ispitivanje pitanja kao \u0161to su:<\/p>\n\n<ul class=\"wp-block-list\">\n<li>Da li nivoi tiroidnih hormona uzro\u010dno uti\u010du na kardiovaskularne bolesti?<\/li>\n\n\n\n<li>Da li funkcija \u0161titne \u017elezde uti\u010de na gojaznost ili metaboli\u010dki sindrom?<\/li>\n\n\n\n<li>Da li je vitamin D uklju\u010den u autoimunske bolesti \u0161titne \u017elezde?<\/li>\n\n\n\n<li>Da li funkcija \u0161titne \u017elezde uti\u010de na zdravlje kostiju ili kognitivni pad?<\/li>\n\n\n\n<li>Da li su cirkuli\u0161u\u0107i biomarkeri uklju\u010deni u razvoj raka \u0161titne \u017elezde?<\/li>\n<\/ul>\n\n<p class=\"wp-block-paragraph\">Umesto da se oslanjaju isklju\u010divo na opservacione asocijacije, MR omogu\u0107ava istra\u017eiva\u010dima da utvrde da li ove veze verovatno odra\u017eavaju stvarne biolo\u0161ke mehanizme.<\/p>\n\n<p class=\"wp-block-paragraph\">Takvi dokazi su posebno vredni pri identifikaciji potencijalnih terapijskih projekta InnoThyroGen, Mendelska randomizacija se koristi za identifikaciju biomarkera i biolo\u0161kih faktora koji verovatno igraju uzro\u010dnu ulogu u bolestima \u0161titne \u017elezde.<\/p>\n\n<p class=\"wp-block-paragraph\">Integracijom velikih podataka studija asocijacije na nivou celog genoma (GWAS) sa naprednim statisti\u010dkim metodama, MR nam omogu\u0107ava da razlikujemo varijable koje su samo povezane sa bole\u0161\u0107u \u0161titne \u017elezde od onih koje verovatnije direktno doprinose razvoju bolesti.<\/p>\n\n<p class=\"wp-block-paragraph\">Ove informacije su posebno vredne za razvoj integrisanog alata za predvi\u0111anje rizika od bolesti \u0161titne \u017elezde koji se razvija u okviru InnoThyroGen projekta.<\/p>\n\n<p class=\"wp-block-paragraph\">Umesto da se biomarkeri biraju isklju\u010divo na osnovu opservacionih asocijacija, MR poma\u017ee u prioritizaciji varijabli sa dokazima o uzro\u010dnosti za uklju\u010divanje zajedno sa poligenskih skorovima rizika (PRS) i klini\u010dkim karakteristikama.<\/p>\n\n<p class=\"wp-block-paragraph\">Kombinovanje genetske predispozicije zabele\u017eene PRS-om sa uzro\u010dno relevantnim biomarkerima ima potencijal da pobolj\u0161a predvi\u0111anje rizika, podr\u017ei raniju identifikaciju pojedinaca sa pove\u0107anim rizikom i doprinese personalizovanijim pristupima prevenciji i le\u010denju bolesti \u0161titne \u017elezde.<\/p>\n\n<p class=\"wp-block-paragraph\">Zajedno, Mendelska randomizacija i poligenski skorovi rizika \u010dine komplementarne komponente analiti\u010dkog procesa InnoThyroGen projekta, pru\u017eaju\u0107i dokaze potrebne za razvoj robusnog i klini\u010dki relevantnog integrisanog alata za predvi\u0111anje rizika.<\/p>\n\n<h2 class=\"wp-block-heading\">Pogled unapred<\/h2>\n\n<p class=\"wp-block-paragraph\">Brzi rast javno dostupnih genetskih skupova podataka transformisao je Mendelsku randomizaciju u jedan od naj\u010de\u0161\u0107e kori\u0161\u0107enih pristupa u genetskoj epidemiologiji.<\/p>\n\n<p class=\"wp-block-paragraph\">Danas istra\u017eiva\u010di mogu ispitati hiljade potencijalnih uzro\u010dnih veza koriste\u0107i podatke iz me\u0111unarodnih biobanaka i kolaborativnih studija asocijacije na nivou celog genoma koje uklju\u010duju stotine hiljada \u2013 ili \u010dak milione \u2013 u\u010desnika.<\/p>\n\n<p class=\"wp-block-paragraph\">Kako se ovi resursi nastavljaju \u0161iriti, Mendelska randomizacija \u0107e igrati sve va\u017eniju ulogu u identifikaciji mehanizama bolesti, vo\u0111enju razvoja lekova i unapre\u0111enju precizne medicine.<\/p>\n\n<p class=\"wp-block-paragraph\">Za projekte kao \u0161to je InnoThyroGen, to zna\u010di prelazak sa identifikacije jednostavnih asocijacija ka razumevanju za\u0161to se bolesti
